If available, can you please send along the incumbent contract or task order number? Response Question 1: F5N3LS1168AW01 procurement is a new requirement, but is a continued acquisition requirement for a recurring services of X-Ray Preventative Maintenance and Repair. The current contract award contract number FA660617P0002 dollar value is $33,300.00. ______________________________________________________________________________________________________ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation Number: F5N3LS1168AW01, is hereby issued as a Request for Quote (RFQ) for the maintenance and repair of an AutoClear X-Ray Unit, Model AC7555ME. This requirement will be fulfilled using utilizing FAR Part 13, Simplified Acquisition Procedures, as a Firm-Fixed Price (FFP) award This acquisition is a 100% small business set aside and is restricted to small businesses only in accordance with FAR 19.502-2(b). The North American Industry Classification System Code (NAICS) is 811219 – Other Electronic and Precision Equipment Repair and Maintenance, Small Business Size Standard is $22 million. (v) Description of Requirement: The Service Provider (SP) shall provide all personnel, equipment, tools, materials, labor, software, and replacement parts to perform preventative/as required maintenance and repairs of AutoClear X-Ray machine: AutoClear Model AC7555ME *See attachment “Performance Statement of Work (PWS)” dated 28 Sep 2021 for additional details* Warranty: Requesting workmanship warranty requirements for a period of one (1) year from inspection and acceptance by the Government. In the event of a manufacturer’s warranty the year will begin at end of manufacturer’s warranty The contract will include FAR 52.217-8, Option to Extend Services, with the potential to extend the contract an additional 6 months beyond the awarded contract performance period. This provisional option, CLIN 5001 is included for evaluation purposes only and will not be included in resultant award. Should an extension of services be required, it may be exercised at the end of any performance period, at the prevailing rates for that performance period. (vii) Acceptance is FOB destination: 439 LRS, Westover ARB, 100 Logistics drive, Chicopee, MA 01022 (viii) Provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Quotes must include the following information: Quotes shall be on company letterhead stationery, Pricing, Solicitation Number, Company Name & Address, Point of Contract, Telephone, Email, Cage Code, DUNS Number, Business Size, terms of any express warranty, any discount terms, Acknowledgement of solicitation amendments, and copy of certification to work on AutoClear equipment. The following FAR clauses shall be completed and returned with quote: Vendors MUST certify provision 52.204-24 and 52.204-26 by completing the attached PDF titled, attachment number “Telecommunications Provisions” and submitting it with their quotes. Vendor Solicitation amendments, if any, need to be acknowledged. (ix) Provision 52.212-2, Evaluation-Commercial Items, applies to this acquisition. (xii) IAW FAR 52.204-16, prospective awardees shall have an active Cage Code prior to award of a government contract.
