This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Shipping will be FOB Destination QUOTATION PREPARATION INSTRUCTIONS To ensure timely and equitable evaluation of quotes, offerors must comply with instructions contained herein. Quotes must be complete, self-sufficient, and respond directly to the requirements of this RFQ. a. The offeror must submit dated quote with unit prices and extended prices in response to this solicitation. In doing so and without taking any exception to the requirements of this synopsis/solicitation, including specification, “Schedule of Supplies/Services”, and terms and conditions, the offeror accedes to all solicitation requirements. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. b. The following factors shall be used to evaluate offers: (i) price (ii) technical acceptability (b) Evaluation is on Lowest Price Technically Acceptable (LPTA) basis. (End of Clause) 252.211-7003 Item Unique Identification and Valuation (Mar 2016) (a) Definitions. As used in this clause— “Automatic identification device” means a device, such as a reader or interrogator, used to retrieve data encoded on machine-readable media. “Concatenated unique item identifier” means— (1) For items that are serialized within the enterprise identifier, the linking together of the unique identifier data elements in order of the issuing agency code, enterprise identifier, and unique serial number within the enterprise identifier; or (2) For items that are serialized within the original part, lot, or batch number, the linking together of the unique identifier data elements in order of the issuing agency code; enterprise identifier; original part, lot, or batch number; and serial number within the original part, lot, or batch number. “Data matrix” means a two-dimensional matrix symbology, which is made up of square or, in some cases, round modules arranged within a perimeter finder pattern and uses the Error Checking and Correction 200 (ECC200) specification found within International Standards Organization (ISO)/International Electrotechnical Commission (IEC) 16022. “Data qualifier” means a specified character (or string of characters) that immediately precedes a data field that defines the general category or intended use of the data that follows. “DoD recognized unique identification equivalent” means a unique identification method that is in commercial use and has been recognized by DoD. All DoD recognized unique identification equivalents are listed at http://www.acq.osd.mil/dpap/pdi/uid/iuid_equivalents.html. “DoD item unique identification” means a system of marking items delivered to DoD with unique item identifiers that have machine-readable data elements to distinguish an item from all other like and unlike items. For items that are serialized within the enterprise identifier, the unique item identifier shall include the data elements of the enterprise identifier and a unique serial number. For items that are serialized within the part, lot, or batch number within the enterprise identifier, the unique item identifier shall include the data elements of the enterprise identifier; the original part, lot, or batch number; and the serial number. “Enterprise” means the entity (e.g., a manufacturer or vendor) responsible for assigning unique item identifiers to items. “Enterprise identifier” means a code that is uniquely assigned to an enterprise by an issuing agency. “Government’s unit acquisition cost” means— (1) For fixed-price type line, subline, or exhibit line items, the unit price identified in the contract at the time of delivery; (2) For cost-type or undefinitized line, subline, or exhibit line items, the Contractor’s estimated fully burdened unit cost to the Government at the time of delivery; and (3) For items produced under a time-and-materials contract, the Contractor’s estimated fully burdened unit cost to the Government at the time of delivery. “Issuing agency” means an organization responsible for assigning a globally unique identifier to an enterprise, as indicated in the Register of Issuing Agency Codes for ISO/IEC 15459, located at http://www.aimglobal.org/?Reg_Authority15459. “Issuing agency code” means a code that designates the registration (or controlling) authority for the enterprise identifier. “Item” means a single hardware article or a single unit formed by a grouping of subassemblies, components, or constituent parts. “Lot or batch number” means an identifying number assigned by the enterprise to a designated group of items, usually referred to as either a lot or a batch, all of which were manufactured under identical conditions. “Machine-readable” means an automatic identification technology media, such as bar codes, contact memory buttons, radio frequency identification, or optical memory cards. “Original part number” means a combination of numbers or letters assigned by the enterprise at item creation to a class of items with the same form, fit, function, and interface. “Parent item” means the item assembly, intermediate component, or subassembly that has an embedded item with a unique item identifier or DoD recognized unique identification equivalent. “Serial number within the enterprise identifier” means a combination of numbers, letters, or symbols assigned by the enterprise to an item that provides for the differentiation of that item from any other like and unlike item and is never used again within the enterprise. “Serial number within the part, lot, or batch number” means a combination of numbers or letters assigned by the enterprise to an item that provides for the differentiation of that item from any other like item within a part, lot, or batch number assignment. “Serialization within the enterprise identifier” means each item produced is assigned a serial number that is unique among all the tangible items produced by the enterprise and is never used again. The enterprise is responsible for ensuring unique serialization within the enterprise identifier. “Serialization within the part, lot, or batch number” means each item of a particular part, lot, or batch number is assigned a unique serial number within that part, lot, or batch number assignment. The enterprise is responsible for ensuring unique serialization within the part, lot, or batch number within the enterprise identifier. “Type designation” means a combination of letters and numerals assigned by the Government to a major end item, assembly or subassembly, as appropriate, to provide a convenient means of differentiating between items having the same basic name and to indicate modifications and changes thereto. “Unique item identifier” means a set of data elements marked on items that is globally unique and unambiguous. The term includes a concatenated unique item identifier or a DoD recognized unique identification equivalent. “Unique item identifier type” means a designator to indicate which method of uniquely identifying a part has been used. (4) Data syntax and semantics of unique item identifiers. The Contractor shall ensure that— (i) The data elements (except issuing agency code) of the unique item identifier are encoded within the data matrix symbol that is marked on the item using one of the following three types of data qualifiers, as determined by the Contractor: (A) Application Identifiers (AIs) (Format Indicator 05 of ISO/IEC International Standard 15434), in accordance with ISO/IEC International Standard 15418, Information Technology – EAN/UCC Application Identifiers and Fact Data Identifiers and Maintenance and ANSI MH 10.8.2 Data Identifier and Application Identifier Standard. PURPOSE: The Government requires additional accessories to the waterjet machining system that was acquired in FY18. The accessories will be attachments to the existing waterjet system. Location of the OMAX Waterjet Machining System is the 908th Maintenance Squadron, 908th Fabrication Flight 1 SOC Drive, Building 1456, Room 114, Maxwell AFB, AL, 36112. •2. EXECUTIVE SUMMARY: In FY18, the Government purchased an OMAX Machining System. At this time we wish to acquire two (2) attachments. The first, is a Tilt-A-Jet, will allow Metals Technology Section personnel to make more precise cuts that would provide closer tolerances when machining aircraft parts, maintenance support equipment and shop equipment components. The secon accessory, a Variable Speed Solids Removal System (VS-SRS) is an environmentally safe system that would avoid exposure to hazardous materials by the same personnel. •3. DESCRIPTION OF REQUIREMENTS: •a. General: The Contractor shall provide all transportation, equipment, labor and materials needed for the following requirement for accessories to an existing OMAX Waterjet Machining System to include: OMAX Tilt-A-Jet, M/N:303390-15 (brand name or equal) and a Variable Speed Solids Removal System (VS-SRS), M/N: 308093 (brand name or equal). The following specifications should be addressed at a minimum: • i. Accessories must be compatible for use with the OMAX Waterjet Machining System, M/N: 5555 AbrasiveJet - 80" x 65" Table w/30hp Pump Machine, Part Number: 5555/30V. • ii. A factory technician will arrive on site to connect OMAX Tilt-A-Jet and Variable Speed Solids Removal System (VS-SRS) to the existing waterjet machining system. •b. Equipment Installation, Set-up, Operational Testing and Training: • i. The Contractor shall perform installation, set-up, system start-up, operational testing and appropriate adjustments of the Tilt-A-Jet and Variable Speed Solids Removal System (VS-SRS) ensuring continued operation of the existing OMAX Waterjet Machining System to make a complete operational system. • ii. Ensure all parts and associated electronic hardware/software are operational. • iii. Utilize existing power and air supply points as necessary. • iv. Contractor shall be responsible for the removal of all debris that accumulated during waterjet system accessory component installations and leave the area in a cleaned condition upon completion of installations. • v. Contractor shall provide onsite operator training to qualified shop technicians to provide a thorough understanding of the basic operations of the Tilt-A-Jet and Variable Speed Solids Removal System (VS-SRS) to be conducted immediately following equipment installation. •4. WARRANTY PERIOD. Contractor shall provide a minimum one year written warranty of parts and labor for the Tilt-A-Jet and Variable Speed Solids Removal System (VS-SRS). During the warranty period, the Contractor shall perform repair maintenance on the waterjet system accessories. •5. GENERAL INFORMATION: •a. Items must be brand new. Refurbished or used items are not authorized •b. Normal Hours. The Contractor's hours are Monday through Friday, from 0700 to 1600 hours or until such a time as the QAE inspection is complete and all discrepancies detected are cleared. •c. Contractor shall coordinate installation dates with Government. Coordination is required to ensure availability of equipment to be installed. Prior coordination with Fabrication Flight Chief is to ensure that the scheduled work load is not impacted during installation. •6. SECURITY REQUIREMENTS FOR UNCLASSIFIED SERVICES ONLY (No CAC Required) •a. Contractor Notification Responsibilities. The contractor shall notify the Information Security Program Manager (ISPM) at Maxwell AFB, Alabama 15 days before on-base performance of work starts. The notification shall include: • i. Name, address, and telephone number of contractor representatives. • ii. The contract number and contracting agency. • iii. The location of where work is performed. • iv. The date work performance starts. •b. Installation Access Requirements: As prescribed by the AFFAR 5352.242-9000, Contractor access to Air Force installations, AFI 31-218, Motor Vehicle Traffic Supervision, and AFMAN 31-113, Installation Perimeter Access Control. •c. The Contractor shall obtain base identification and vehicle passes for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished identification bafges while visiting or performing work on the installation. •d. The Contractor shall submit a written request on company letterhead to the Contracting Officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The individual(s) should provide a valid driver's license, current vehicle registration, andvalid vehicle insurance certificate to obtain a vehicle pass. Government ID card holders may not use their credentials to sponsor an individual on to the base to perform work that's been contracted to an official Contractor. •e. During performance of work, the Contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. •f. All commercial vehicles will be directed to Gate 3 (Kelly St) or Gate 4 (Congressman Dickenson for Gunter Annex) for processing through the Commercial Vehicle Inspection (CV1)Area, when operational During non-duty hours and holidays, commercial vehicle inspections will be conducted at Gate I (Maxwell Blvd) or Gate 4 only. Inspection members will conduct thorough inspections of the interior and exterior of the vehicle for items prohibited from the installation such as explosive devices, weapons and ammunition, drugs, and open or closed alcohol containers. •g. Upon completion or termination of the contract or expiration of the identification passes, the prime Contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. •h. Failure to comply with these requirements may result in withholding of final payment. •7. Traffic Laws: The Contractor and their employees shall comply with base traffic regulations set forth in AFI 31-218, Motor Vehicle Traffic Supervision and AFMAN 31-113, Installation Perimeter Access Control. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket. •8. Random Installation Entry/Exit Checks: Entry/exit vehicle checks are conducted by order of the 42 AB W Commander. These checks are conducted for the purpose of safeguarding the base and protecting government property by discovering and seizing stolen property, classified information and contraband. Refusal to submit to an installation (entry/exit) vehicle check may result in the loss of base driving privileges, revocation of base registration, or debarment action. •9. Cellular Phone Operation Policy: The use of cellular phones while operating a motorized vehicle is prohibited on Maxwell-Gunter. Although discouraged, drivers are authorized to use devices, i.e. ear bud or ear boom, which allows their cellular phone to be operated hands-free. The device must not cover both ears. This policy applies to everyone driving on Maxwell AFB and Gunter Annex. Personnel in violation may be issued a Central Violations Bureau Form 1805 traffic ticket. •10. Wireless Electronic Devices: The Contractor shall not establish his/her own Information Technology (IT) systems or networks (Local Area Networks [LAN], Wide Area Network [WAN], cellular phone/USB modem as WAN, Wi-Fi as WAN, etc.), or camera system without tbe direct permission of the Information Security Program Manager and governing communications and responsible information systems office (42 CS). •11. Firearms and Ammunition: Transporting weapons or ammunition, concealed or otherwise, IS NOT permitted by any non-law enforcement personnel on Maxwell AFB/Gunter Annex at any time regardless of state issued concealed weapons/LEOSA permits. Violations may result in criminal prosecution under the applicable federal laws. •12. Illegal Weapons: The below weapons are considered illegal, unless specifically authorized by competent authority, and are prohibited on Maxwell AFB/Gunter Annex. Violations may result in criminal prosecution under the applicable federal laws. •a. Switchblade knives or knives with any type of automatic blade release. •b. An incendiary/explosive weapons (e.g. grenades, flash bangs). •c. Fireworks •d. Homemade mortars, aka "tennis ball launchers" or similar devices. •13. HEALTHCARE. Healthcare provided at the local military treatment facility on an emergency reimbursable basis only. •14. HOLIDAYS.
