The contractor shall furnish all labor, tools, parts, materials, and facilities, necessary to provide repairs for all equipment listed in Appendix 5 of the PWS. The equipment is currently located at the United States Air Force (USAF) Hospital, 633d Medical Group (633 MDG), Joint Base Langley-Eustis (JBLE), Virginia , Langley AFB division, also referred to as the Military Treatment Facility (MTF) herein. All equipment from Appendix 5 will be shipped to the contractor upon needing repair. All labor necessary to perform installation of replacement parts, and other servicing of equipment shall be included in the contractor's pricing to repair the system and ancillary equipment. 1.0.1. SCOPE INVENTORY. When necessary, MTF may add or remove items from listed chart, indicated in Appendix 5, for no additional charge. Addition of 10 or more items, with no removal, may be subject to discussion of additional charges. 1.0. REQUESTS FOR SERVICE. Only the Contracting Officer (CO), Contracting Officer Representative (COR), Functional Requirements Evaluator Designee (FRED) or Medical Maintenance personnel are authorized to place requests for service on this contract. DESCRIPTION OF SERVICES Unscheduled/Corrective Maintenance. 1.1.1.2. All required parts and service shall be furnished as part of this contract without additional cost to the government. The CO, COR or FRED in Medical Maintenance has the authority to place a service call with the contractor, which will the contractor shall respond to within 24 hours during the hours stated in 1.2.4.1. Maintenance actions. Once a maintenance action is initiated, it shall be continuous until the task is complete. Medical Maintenance will ship out the damaged equipment once contact has been made with the contractor. Compliance shall be manifest by continuous presence of the contractor on the job site, unless waiting for delivery of replacement parts. Under no circumstances shall maintenance actions be delayed by more than 48 hours while waiting for delivery of parts, unless the parts cannot be currently obtained or are on back order. Parts. The contractor shall furnish and replace parts to meet up-time requirements. The following requirements apply to parts provided under this contract. The contractor shall have ready access to unique and/or high mortality replacement parts. All parts supplied shall be compatible with existing equipment. All parts used for service of this system shall be new, of first quality, and approved for ' use by the OEM. Used parts, those removed from another system, shall not be installed without prior approval of the CO, COR/Medical Maintenance. Service Manuals. The government will not provide service manuals or service diagnostic software to the contractor. The contractor shall be responsible for providing all service manuals, operational manuals, schematics, and diagnostic software necessary to maintain the required system up-time to the contractor's field service representative(s). Updates. The contractor shall provide all updates, to include: installing manufacturer designed and issued field modifications, safety corrections and revisions for correcting software and equipment deficiencies at no additional cost to the government. Upgrades and modifications shall be furnished at no additional cost under the contract and installed during the PMis after the manufacturer makes them available. Updates shall not include those designed primarily as performance enhancements or performance upgrades. Conformance Standards. The contractor shall keep the system in good operating condition, meeting all equipment and performance requirements published by the OEM. The contractor assumes responsibility for system conformance with the latest published edition of National Fire Protection Association (NFPA) 99, Occupational Health and Safety Administration (OSHA), and Center for Devices and Radiological Health (CDRH) standards. GENERAL INFORMATION Personnel. Point of Contact. The contractor shall provide a primary, and alternate, point of contact in writing, at time of contract award, with full authority to interface with the government representative or CO on all contract matters relating to the daily operations. If the primary point of contract should change, the contractor shall, within 15 calendar days of the change, notify the CO in writing. Qualifications/Education/Training. Prior to award of the contract and each subsequent performance period, the contractor shall certify to the FRED that the field service representative has had professional formal training on equipment specific to this contract, and at least annual continuing education, to include OEM specific training. If the training occurred more than three years ago, proof of recent (during past 18 months) maintenance experience on specified equipment must be provided. The FRED/Medical Maintenance department may at any time request verification of service personnel credentials who are servicing the equipment from the contractor. Hours of Coverage. Normal hours of coverage are, Monday through Friday, 0730-1630 hours, excluding Federal holidays listed in paragraph 1.2.4.2. below and additional days, as designated by the government, where the hospital will be closed. United States Federal holiday observed by the US Government include: New Year' s Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Response Time. The contractor will make every effort to respond with a phone call to the FRED/Medical Maintenance department or his/her designee within four (4) working hours after telephone notification during normal hours of coverage. Scopes will be repaired and returned within 14 calendar days with day 1 being the day the scope is received. Administrative Control. All maintenance documents shall be prepared and validated in triplicate. The system user, FRED/Medical Maintenance department, and the contractor shall each retain a copy. At least one copy of the report must be placed in the package upon it being returned to the MTF. Documentation and reporting shall be in compliance with the Joint Commission standards. The System Maintenance Record for each service call will include at minimum: Name of the contractor. Name of the contractor's field service representative. ·1.2.7.4. Date, time (starting and ending), equipment downtime, and hours-on-site. The contractor' s field service representative must report to the Medical Maintenance department in accordance with local guidelines. Description of the problem reported by the FRED/Medical Maintenance department/user , and an itemized description of service performed (including labor, parts, and corrective action . , taken). Identification of equipment to include equipment control number, manufacturer, device, model number, and serial number. Billing Procedures. This service contract shall be billed monthly, in arrears. The contractor's invoice shall reflect the following information: Invoice number Contract/purchase order number. Billing period. Amount in American dollars. Manufacturer, model and serial number of each piece of equipment serviced with documentation of work performed or part(s) provided. Invoices shall be submitted in accordance with (IAW) wide area workflow (WAWF) procedures. · Certification of payment will not be accomplished until all required service documents have been received by the FRED/Medical Maintenance department for the payment period. Claims of government misuse or abuse. If the contractor claims any service calls are the result of government abuse or misuse, the damage shall immediately be shown on the system maintenance record, verified by the FRED/Medical Maintenance department, and approved by the CO before repairs are made. The CO will resolve disagreements as to responsibility between both parties. Reporting. Required services beyond the scope of contract. The contractor shall immediately, but not later than 24 consecutive hours after discovery, notify the CO and FRED or Medical Maintenance department in writing of the existence or the development of any defects in, or repairs required to the scheduled equipment for which the contractor considers he/she is not responsible under the terms of the contract. The contractor shall furnish the CO and FRED/Medical Maintenance with a written estimate of the cost to make necessary repairs, and a full technical description, prior to the commencement of work. Additional charges. There shall be no additional charge for time spent at the site awaiting the arrival of additional contractor personnel and/or delivery of parts. Safety. The contractor shall comply with American OSHA Standards, and Air Force safety standards at all times while on government property. Safe working practices shall be followed. Requirements include, but are not limited to: The government's use of lock-out/tag-out procedures while working on electrical systems and equipment. Report any injuries to contracted employees or damage to government equipment to the COR/Medical Maintenance department at 757-764-6785, immediately. Security Requirements. The contractor shall be responsible for safeguarding all government property and controlled items provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured by government personnel. Contractor personnel shall not be issued keys to any facilities by government personnel. Performance of Services during Labor Strikes. If the contractor personnel are under a Collective Bargaining Agreement, the contractor shall submit an emergency strike plan no later than-ten (10) workdays after start of contract. Smoking in Air Combat Command (ACC) Facilities. Contractors and/or contractor personnel are advised that the ACC Commander has placed restrictions on the smoking of tobacco products in ACC facilities. AFI 40-102; Tobacco Use in the Air Force (current edition), outlines the procedures used by the commander to control smoking in our facilities. Contractors, contractor personnel and visitors are subject to the same restrictions as government personnel. Smoking is permitted only in designated smoking areas. Traffic Laws. The contractor and/or contractor personnel shall comply with the installation traffic code as specified in Virginia State Law and Langley Air Force Base Instruction (LAFBI) 31-204, Langley Air Force Base Traffic Code (current edition). The contractor and/or contractor personnel shall comply with current instruction on cell phone use while on a military installation. Weapons, Firearms, and Ammunition. The contractor and/or contractor personnel is/are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on any Air Force base. SECTION 2 SERVICES SUMMARY (SS). The contractor service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimal acceptable levels of service required for each requirement. The SS and the contractor's Quality Control Plan (QCP) provide information on contract requirements, the expected level of the contractor's performance and the expected method of government surveillance and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the Federal Acquisition Regulation (FAR) 52.212-4 (a), Contract Terms and Conditions- Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies. The absence of any contractor requirements from the SS shall not detract from its enforceability nor limit the rights or remedies of the government under any other provision of the contract, including the clauses entitled "Inspection of Services" and "Default." The government has the right to inspect all services required in the contract.
