AMENDMENT #1: This Notice is to revise that ASRC Federal Field Services will be contemplating an award for a Linen a Rental Service for a base period beginning between 1 April 22 - 15 April 22 through 31 March 23 with a 2 to 6 year annual option. The 71st FSS at Vance AFB, OK is contemplating an award for a Linen Rental Service for a base Period of 01 Apr 22 through 31 Mar 23 plus two (2) annual options. This is a commercial purchase using Federal Acquisition Regulation (FAR) Part 12 and Part 13 procedures. The following information, as required per FAR 12.603(c)(2) applies: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested. (ii)The solicitation reference number CB-BL0422-BK; it is being issued as a Request for Quotation (RFQ). The Location for Greven Crosswinds Club is 454 Fisher Road, Bldg 601 and Falcon's Nest Bowling Center is 308 Sawyer Rd, Vance AFB, OK. 2.SERVICE TO BE ACCOMPLISHED: Deliveries will be accomplished weekly Monday thru Friday asrequired between the hours of 9:00am-5:00pm. Service provider shall provide: a.A verifiable method of tracking items returned for service. b.A verifiable method of tracking invoice totals against contract amount. c.A single point of contact (POC) responsible for resolving customer complaints, changes, and problem resolution, maintaining a 95% satisfaction average rating for matters concerning Greven Crosswinds Club and Falcon’s Nest Bowling Center. At the time delivery 10% of the bundles delivered will be inspected for cleanliness. d.SEE BELOW for annual estimates of linen quantities, colors, and sizes. Fabric is a 50/50 cotton polyester blend. All table linens are owned by the service provider with the exceptionof the banquet chair covers and table skirts. The Greven Crosswinds Club owns 250 ivory banquet chair covers and 16 table skirts in assorted colors which will require cleaning and pressing as needed. e.Linen shall be delivered on Thursday, at that same time all existing clean and dirty linen from the previous week shall be picked up by contractor. i.Contractor is responsible for ensuring adequate vehicles are available to deliver and pickup all linen. 3.SATISFYING REQUIREMENTS FOR INSTALLATION ACCESS: Service provider is responsible for satisfying and sustaining the requirements for base access throughout, but not to exceed, the duration of performance. a.Instruction for Vance AFB Installation Access Application: The contractor's information goes into the primary contractor box. The portion of the application from "name" all the way to "signatureof applicant" is completed by the person needing access to the base. After they complete that portion, you or whatever the CAC holder is requesting access will complete the bottom blue/gray portion of the application and return it to the Visitor's Center. When the application is approved ordenied, the person will be notified to pick up their card. The checks are good for 365 days and the card can be made for that time span. b.SEE ATTACHMENT: B 4.DETAILS: Phone number for POC shall be toll free. POC e-mail access is also preferred. Service provider shall accomplish all invoicing and problem resolution directly with the AFFS Local Purchase at Vance AFB.The Service provider shall obtain receipt of service signatures on all delivery orders. Any resulting ServiceContract shall be subject to applicable SCA wage determination. Greven Crosswinds Club and Falcon’s Nest Bowling Center is a Government facility and shall be tax exempt. Government personnel reserve theright to inspect contractor’s facilities. 5.PROPOSED DELIVERIES: Options shown have been extended to a full twelve-month period. These options are provided to give Service provider as an idea of the options that may be required annually.Orders are placed weekly and may be a combination of colors, depending on the event scheduled.
