THIS IS NOT A REQUEST FOR QUOTE/PROPOSAL - Pursuant to FAR 5.203(a), Washington Headquarters Services, Acquisition Directorate (WHS/AD), on behalf of the Department of Defense 4th Estate intends to award a Hybrid contract on a sole source basis to Millenium Health and Fitness, Inc., 28170 N Alm School Pkwy Ste 204. The current contract ends on November 9, 2019. The period of performance is estimated to be for one-twelvemonth base period. The anticipated period of performance is 10 November 2019 to 9 November 2019. The estimated range of the one year bridge is between $1,000,000.00 - $5,000,000.00. The anticipated award will be made under the authority of FAR 6.302-1(a)(2)(iii), "Only one responsible source and no other supplies or services will satisfy agency requirements." The contract is intended to meet the requirement to continue Employee Assistance Programs and Work Life Support Services with various 4th Estate agencies.
