The contractor shall provide a qualified neuromonitoring technologist to be dispatched and be present in the operating room to perform intra-operative neuromonitoring (IONM) at Walter Reed National Military Medical Center (WRNMMC) located in Bethesda, MD. The technologist shall function as an integral member of the neuromonitoring team by providing consultation services to the Surgical Neurophysiologists during surgery. The Contract Technologist performs pre/post- and real-time on-site intra-operative monitoring at WRNMMC. All contractor employees shall meet the qualifications for their specialty and perform their roles and functions as stated in the PWS. The contractor must be located within 50 miles of WRNMMC due to the on-call nature of the work. The contractor shall provide all equipment and maintenance thereof to perform services; the only supplies provided will be monitor leads. See Performance Work Statement attachment. INSTRUCTIONS FOR PROPOSALS: 1. Offerors shall populate the "Price Matrix" attachment and include it with their proposal submission 2. Offerors shall include no more than three (3) past performance references that are relevant to this requirement. Reference information shall include, at minimum: contract number, customer name, customer e-mail address and/or phone number, contract dollar amount, and a brief description of work performed.
