The contractor shall provide AMBULANCE SERVICES to Walter Reed National Military Center located in Bethesda MD. This requirement for a non-personal services contract to provide Ambulance Services with health care support in strict compliance with standing orders and protocols, set forth by the MTF for Ambulance services, which include providing the appropriate combination of certified/licensed Emergency Medical Technicians Basic (EMT-B), Cardiac Rescue Technician (CRT), CRT Intermediate (CRT-I), EMT-Paramedics (EMT-P), and /or Critical Care Transport Nurses to meet patient transport category designated by the requesting facility. See the Performance Work Statement attachment. INSTRUCTIONS FOR PROPOSALS: Please review newly attached 52.212-1 Instructions to Offerors Commercial Items Instructions and Addenda to Clause 52.212-2 Evaluation Commercial Items, Past Performance Summary Attachment and Past Performance Questionnaire. 1. Offerors shall populate the " Price Matrix” attachment and include it with their proposal submission 2. Offerors shall include no more than three (3) past performance references that are relevant to this requirement. Reference information shall include, at minimum: contract number, customer name, customer e-mail address and/or phone number, contract dollar amount, and a brief description of work performed.
