This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, and FAR part 13.106-1(b)(1), Soliciting from a single source. The Defense Health Agency (DHA) intends to negotiate on a sole source basis in accordance with FAR part 13.106-1(b)(1). The proposed source is Doximity, Inc., 500 3rd Street, Suite 510, San Francisco, CA 94107-6803. This acquisition is for the procurement of twenty-nine (29) AMiON Enterprise Software Licenses, including all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware utilized by the 633rd Medical Group (MDG), Joint Base Langley-Eustis, VA. This acquisition is for 29 AMiON enterprise software licenses, including all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware utilized by the 633rd Medical Group (MDG). The vendor shall provide only AMiON enterprise Original Equipment Manufacturer (OEM) certified and qualified technicians authorized to perform services on the AMiON software and hardware. The vendor shall provide all-inclusive labor, equipment, tools, parts, materials, and preventative maintenance (PM) and corrective maintenance (CM) support necessary to ensure proper functionality of the AMiON enterprise software and/or hardware. Services will be performed at the 633rd MDG, Joint Base Langley-Eustis, VA. Contractor shall be OEM authorized dealer, authorized distributor, or authorized reseller for the proposed software/hardware and equipment such that OEM warranty and services are provided and maintained by the OEM. All software licensing, warranty, and services associated with the software/hardware and equipment shall be in accordance with OEM terms and conditions.
