SUBJECT: Combined Synopsis/Solicitation for; IMAM Pastoral Services; Walter Reed National Military Medical Center 1.) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2.) The Solicitation ID for this requirement is 0011858524, This Requirement is being solicited as a Request for Quotes (RFQ). 3.) This solicitation document incorporates provisions and clauses from Federal Acquisition Regulation (FAR), FAC 2023-01; and supplemental Regulations from the Defense Federal Acquisition Regulation Supplement (DFARS), DFARS Change 03/01/2023. 4.) This requirement is being solicited under full and open competition under the NAICS code 813110 5.) In order to be considered for this requirement vendors must submit two past performance questionnaires accomplished by two separate entities and completed the Past Performance Summary. (See Attachment) 6.) The table below provides the itemized list of materials required for this requirement, interested parties shall fill the table with applicable information as it pertains to this requirement; i.e Unit price, extended price, estimated delivery, etc. (PLEASE SEE ATTACHED COMBO) 7.) Offers are due 29 March 2023, by 1300 EST. 8.) The provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition 9.) The provision 52.212-2, Evaluation-Commercial Products and Commercial Services is incorporated in this requirement as prescribed IAW FAR 12.301(C). The government will award a contract resulting from this solicitation to the offeror whose offer conforming to the Solicitation will be most advantageous to the government. The government will evaluate quotes based on price, technical acceptability, and conformity to the solicitation. Additionally, interest parties are required to submit two Past Performance Questionnaires and complete the past performance summary. 10.) Potential offerors must provide a complete copy of the provision 52.213-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with their offer. 11.)
