A market survey is being conducted to gain knowledge of potential qualified small business sources to determine if they can effectively compete for award of a Firm Fixed Price (FFP) contract with a base year and four option years. The U.S. Department of Agriculture’s (USDA) Food Safety an Inspection Service (FSIS), anticipates future procurement requirements for an established provider of Occupational Health Physician and Medical Services with national reach to include remote and rural areas. The company must have: a staff of board-certified occupational health staff to provide annual audiometric examination services for an estimated 7,800 FSIS HCP participants in accordance with OSHA 29 CFR 1910.95. the ability to provide audiometric examination services USDA’s ten districts to include rural and remote locations. The districts are listed below District 1 – Arizona, California, and Nevada District 2 – Florida, Georgia, Puerto Rico, South Carolina, and the U.S. Virgin Islands District 3 – Illinois, Indiana, Michigan, and Ohio District 4 – Louisiana, New Mexico, Oklahoma, and Texas District 5 – Alaska, American Samoa, Colorado, Guam, Hawaii, Idaho, Northern Mariana Islands, Montana, Nebraska, Oregon, Utah, Washington, Wyoming District 6 – Iowa, Minnesota, North Dakota, South Dakota, Wisconsin District 7 – Alabama, Kentucky, Mississippi, Tennessee District 8 - Connecticut, Massachusetts, Maine, New Hampshire, New York, Pennsylvania, Rhode Island, Vermont District 9 – Delaware, District of Columbia, Maryland, North Carolina, New Jersey, Virginia, West Virginia District 10 – Arkansas, Kansas, Missouri It is the intent of this effort to have audiometric examination examinations be made available to federal employees at all FSIS locations. There are approximately 175 field locations with employees needing services within the contracted areas. Services to be rendered by the contractor include but not limited to the following: Audiometric Tests Audiometric tests must include the following: Individual testing of right and left ears via air-conduction, pure-tone, hearing threshold measures at no less than 500, 1000, 2000, 4000, and 6000 hertz (Hz); the 8000 Hz threshold will be tested whenever the etiology of a hearing loss may be in question Testing must be conducted with audiometers which meet the specifications of and are maintained per the American National Standard Specifications for Audiometers, ANSI S3.6 Testing room ambient noise levels must conform to the American National Standard Maximum Permissible Ambient Noise Levels for Audiometric Test Rooms, ANSI S3.1 All audiograms obtained must meet the requirements of OSHA 29 CFR 1910.95, Occupational Noise Exposure, Appendix C: Audiometric Measuring Instruments A technician that performs audiometric tests must be responsible to an audiologist, otolaryngologist, or physician. Use of microprocessor-based or self-recording audiometers does not waive the qualification requirements for the tester. The professional notation must be recorded on each worker’s audiogram. Record Keeping The vendor shall have an electronic record keeping database that stores all MP Region’s audiogram tables or audiometric test charts as specified in 29 CFR 1910.95(m). For each employee tested, the database must provide a historical record that includes: Employee’s name Employee’s date of birth Job classification as of date of most recent audiogram Date and time of test Examiner’s name Date of last acoustic or exhaustive calibration of the audiometer Evaluation comment Hearing thresholds Brief indication of any significant problems noted by employee at time of test This is a Market Research and Sources Sought Notice only. The primary North American Industrial Classification System (NAICS) Code applicable to this requirement is 621340 “Offices of Physical, Occupational and Speech Therapists, and Audiologists.” The size standard for which is a concern, including its affiliates whose average annual gross revenue does not exceed $8.0 million for the past three fiscal years. Consideration shall be given in response to the following: 1). Limitation of Subcontract - Under Federal Acquisition Regulation (FAR) guidelines [48 CFR 52.219-14(b) (1)], the prime contractor must have the capability to perform at least 50% of the contract work with his/her own employees.
