The US Forest Service WO-AQM Job Corps Support Branch has requirement for Onsite Medical Services for the Great Onyx Job Corps Center, Mammoth Cave, KY. The required services include a Physician and/or a Nurse Practitioner/ Physician Assistant. This requirement is being solicited under FAR 13 – Simplified Acquisition Procedures (SAP) as authorized by FAR 13.000, with the intent to award a Firm-Fixed Price Contract. One (1) award will be made, in accordance with the acquisition procedures established in 13.106-2 (b). The Government will award a contract resulting from this solicitation to the responsible Quoter whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The purpose of this synopsis is to gain knowledge of interest, capabilities and qualifications of various members of Small Business, 8(a), HUBZone, Service-Disabled Veteran-Owned Small Business (SDVOSB), and Women-Owned Small Business (WOSB) to compete and perform service under a contract for the attached Performance Work Statement. A Request for Quote (RFQ) solicitation and firm fixed priced contract is contemplated depending upon the responses to this synopsis. This synopsis summarizes the requirements for the contractor to provide basic medical services at Great Onyx Job Corp Center (JCC) in Mammoth Cave, KY. Great Onyx JCC is located at 3115 Ollie Ridge Road, Mammoth Cave, KY 42259. The contractor shall perform basic medical services at the Great Onyx JCC in accordance with the attached DRAFT Performance Work Statement.
