The solicitation is amended to show that a total of 200 hours per year are required. If the state in which the services are being performed requires a supervising physician, then a copy of the collaborative agreement between the PA/NP and the physician must be provided with the quote. All costs associated with providing services must be included in the quoted hourly rate. This includes the costs of a supervising physician, if required/applicable. Since only 4 hours per week are required, services would be performed one weekday per week during normal working hours. As stated in the solicitation, the day of the week is subject to mutual agreement. The US Forest Service WO-AQM Job Corps Support Branch has requirement for Medical Services at the LBJ Job Corps Center; Franklin, NC. The required services will include Physician Services and/or Physician Assistant/Nurse Practitioner Services. The complete solicitation package will be attached and made available only through the FedBizOpps (FBO) website at https://fbo.gov/ on or about December 19, 2018. Anyone wishing to participate in this proposed procurement must obtain their copy from FBO. Copies will not be mailed from the Contracting Office. All amendments will be posted on fbo.gov Contractors will be solely responsible for obtaining amendments from this site to update files. This requirement is being solicited under FAR 13 - Simplified Acquisition Procedures (SAP) as authorized by FAR 13.000, and is a Small Business set-aside, with the intent to award a Firm-Fixed Price Contract. One (1) award will be made, in accordance with the acquisition procedures established in 13.106-2 (b). The Government intends to award a contract resulting from this solicitation to the responsible quoter whose quote, conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical, Past Performance, Price. NOTE 1: All prospective contractors must be registered in the System for Award Management (SAM) database at http://www.sam/gov/ before award of contract; failure to register in the SAM database may cause your firm to be ineligible for the award and removed from consideration. NOTE 2: Notice to Quoters: Funds are presently available for this effort. The Government reserves the right to cancel, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse a quoter for any costs.
