The contractor supplies the alternate veterinarian. Contractor is responsible the background check of all veterinarians. Clarification 2: DEA license not required, contractor or employees will seek product by prescription. 1.3.1 Clarification 1: 30 minute emergency response time is required for primary and alternate veterinarian. Clarification 2: Minimum for on-site rounds is 3 days/week (2 days is a typo) 1.4.1 Clarification: Primary and alternate local project manager can be the same person. 1.6.1 Clarification: A federal employee is required to escort the contractor at all times while on site. 1.6.2 Clarification: The government does not have an estimated cost for background investigations. 2.2.1 Clarification: Only an approved federal employee will conduct euthanasia. 3.3 Clarification: The contractor is not responsible for government vehicle insurance. General Clarifications: • In the event of an animal referral to an offsite clinic, a GRL technician will manage transportation for the animal. • Retention of GRL medical records is allowed. • Federal employees will assist the contractor in animal handling needs. • Any GRL purchased drugs will be transferred to an GRL designated VMO upon expiration of contract. End 28MAR2022 Clarifications This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR subpart 12.06, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 12805B22Q0116 and is issued as a request for quotation (RFQ). This solicitation document incorporates those provisions and clauses in effect through Federal Acquisition Circular 2022-04. The associated NAICS Code is 541940. The USDA, Agricultural Research Service, Plains Area Grazinglands Research Laboratory requires Veterinary Services, Base + 3 six month option periods. See Attachment 001 for more detail. The Government intends to award a firm fixed price purchase order. The requirements are further detailed in the attached Requirements Document. MUST INCLUDE PRICING FOR ALL OPTION PERIODS. (a)Pricing document submission indicates acceptance of the noted specifications in Attachment 001. (b)In addition to pricing towards requirement specifications, document information MUST include offeror Quote Reference Number, Quote Expiration Date, Business Name, DUNS or ueiSAM Number, and Quote Point of Contact (name and email address.) 2) Have an Active SAM.gov registration and complete the Representations in FAR 52.204-54; the Representation and Certification section should also be completed electronically in the System for Award Management (SAM) at www.sam.gov. Any amendment and documents related to this procurement will be available electronically at the Government Point of Entry (sam.gov). FAR clauses 52.212-1, 52.212-2, 52.212-3, 52.212-4 and 52.212-5 (and any addenda to those clauses) are included in the attached clauses and provisions document. The Government anticipates award of a contract resulting from this solicitation to the responsible offeror whose quote conforms to the specifications that will be the most advantageous to the Government in accordance with FAR 13.106-2 considering three major areas: technical acceptance, past performance and price. Quotes will be evaluated by performing a comparative evaluation in accordance with FAR 13.106-2(b)(3) to determine which quote represents the Lowest Price Technically Available. See attached clauses and provisions document for more details. All sources wishing to quote shall furnish a quotation by 11AM Mountain Time, on or before March 31st, 2022.
