This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is attached. The solicitation number is 12805B21Q0036 and is issued as a request for quotation (RFQ). This solicitation document incorporates those provisions and clauses in effect through Federal Acquisition Circular 2005-101. The acquisition is a 100% Small Business Set-Aside. The NAICS code is 811213, business size standard is $12.0 million. The USDA, PA, Southern Plains Agricultural Research Center (SPARC), 2881 F&B Road, College Station, TX 77845 has a requirement for ShoreTel Premier VoIP/PBX and AudioCodes Monthly Full Support Maintenance. This requirement consists of 5 Contract Line Items for services for a base year and 4 (12 month) option periods. The bid schedule, performance work statement, service wage determination, are included as attachments to this notice. In order to be considered for award, offeror shall provide: 1) One (1) original fully completed and signed SF 1449 Offer page, (complete block 30), acknowledgment of all SF30 amendments by completing blocks 15a, b, and c of the Amendment SF30 and attaching to the SF 1449 Offer page; 2) Quote pricing document. Document shall provide detailed pricing breakout for the base year and each option year in accordance with the CLIN structure and items requested. 3) Bid Schedule (MS Excel Interactive Document) Attachment 1. Offeror shall provide the completed Attachment 1 with monthly pricing. 4) Offeror must submit documentation that they are an authorized sales and service support provider for both ShoreTel/Mitel and AudioCode devices. Government will award a firm-fixed price contract using Lowest Price Technically Acceptable (LPTA) evaluation method.
