This is a combined / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Quotes submitted in response to this solicitation shall be furnished in the following format with the numbers of copies as specified below: Quotes shall be submitted in two parts - a technical proposal and a price proposal. Each part shall be separate and complete in itself so that evaluation of one may be accomplished independently of the other. The price quote shall be submitted with the solicitation forms, a separate breakdown of the unit price is required. The technical proposal must not contain reference to cost; however, resource information (such as data concerning labor hours and categories, materials, subcontracts, etc.) must be contained in the technical proposal so that the contractor’s understanding of the statement of work may be evaluated. The form attached titled “Response to Evaluation Factors” is to assist in proposal preparation. Use this form OR include similar information with the technical proposal. Please ensure that all required information is included in your proposal, use additional sheets or formats if necessary. Offerors are cautioned that sufficient and detailed information must be presented in their technical proposals to enable the Government to evaluate their proposals fully in accordance with the evaluation and award criteria contained herein. Technical Proposal. 1. Technical Approach Provide an Operations Plan with a general project information narrative that specifically includes the following information. The technical approach should be in as much detail as the offeror considers necessary to fully explain the proposed technical approach or method. The technical proposal should reflect a clear understanding of the nature of the work being undertaken. Description and breakdown of the type of service to be performed, labor hours and type of worker/s for a typical week at each site. (2) Description of the schedule general plan and schedule by item, by unit. (3) Description of how operations will be performed safely. How you will meet Quality Standards. Capability Information Form (attachment to Evaluation Factors). Relevant Past Performance – Offerors shall submit past performance information for their firm and any proposed major subcontractors performing more than 20% of the work. Performance information may be obtained from references provided. The Offerors shall provide Past Performance information” for all relevant projects completed during the past three years and all contracts and subcontracts in progress. The list should include, but is not limited to, contracts and subcontracts similar in nature and complexity to that required by this solicitation. Include contracts with the Federal Government, agencies of state, and local governments, and commercial customers. Each offeror will be evaluated on their performance under existing and prior contracts of a similar nature. Performance information will be used as an evaluation factor against which offerors' relative rankings will be compared to assure best value to the Government. The Government will focus on information that demonstrates quality of performance relative to the size and complexity of the procurement under consideration. The Government may contact references beyond those identified by the offeror to obtain additional information for use in both the responsibility determination and in further evaluation of the offeror’s past performance. The Office of Management and Budget (OMB) has authorized Federal agencies to collect past performance information under OMB clearance number 9000-0142. Cost/Price: A firm’s completion and submission of unit Price and total price on page 1 of solicitation constitutes the fulfillment of the Government’s price proposal requirements under this solicitation. FAR 52.216-1 TYPE OF CONTRACT (APR 1984) The Government contemplates a firm fixed price contract with a single award resulting from this solicitation. NOTE: The Government assumes no responsibility for any conclusions or interpretations made by the Contractor based on information made available by the Government. Nor does the Government assume responsibility for any understanding reached or representation made concerning conditions which can affect the work by any of its officers or agents before the execution of a contract based on this solicitation, unless that understanding or representation is expressly stated in the contract. EVALUATION FACTORS FOR AWARD 52.212-2 -- Evaluation -- Commercial Items (Oct 2014) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate the offers: Technical, Past Performance, and price. Technical and past performance, when combined are approximately equal to price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Include the sequence and methodology of accomplishing the work items. Address both completion of work within individual units and a general plan for sequence of work across the project, including all work items. 1B. Key Personnel. Provide the resumes of the persons to be designated as the Overall Project Manager and individual operators/sub managers. Include list of successfully completed projects within the last three years, including reference names and phone numbers for these projects. 1C. Equipment. Specify the equipment to be used to accomplish work. Include make and model of proposed equipment. Address the need for multiple pieces of equipment. 1D. Quality Control Plan. Address your proposed Quality Control plan. What supervisory and instruction measures you will employ to ensure that contract specifications will be met prior to requesting a Forest Service inspection? What are your self-inspection measures, including how you will inspect work for compliance with specification? Fully explain how project management/oversight will be accomplished for this project. 1E. Safety Plan. Safety Plan specific to the work site. Mitigation of risks. Communications, actions in cases of emergency. FACTOR 2. PAST PERFORMANCE. Furnish a list of three projects of relevant and similar magnitude completed by the firm within the last five years.
