The U.S. Department of Agriculture (USDA) Animal and Plant Health Inspection Service (APHIS) is soliciting quotes for occupational medicine services in support of the occupational health program at the National Bio and Agro-Defense Facility, Manhattan, KS. Work involves providing professional medical services on an as-needed basis to approximately 70 workers. All services shall be performed at a contractor facility located within 30 miles of the NBAF facility located in Manhattan, KS. The contractor shall provide all necessary labor, facilities, supplies, and equipment to perform the services. The contractor must be Kansas state licensed. The laboratory must be Clinical Lab Improvement Act (CLIA) certified. The period of performance is 12 months. See attached Statement of Work for details. This procurement is unrestricted. The Government anticipates awarding a firm-fixed-price Blanket Purchase Agreement. The appropriate NAICS code for this procurement is 621111 with a small business size standard of $12.0M. Types of medical testing include 12-Lead Resting Electrocardiogram, Audiometry, Chest Radiograph, Pulmonary Function Test, Tuberculin Skin Test, Vision Screening, and Q Fever Antibody test. Vaccinations include Tuberculosis, Hepatitis A, Hepatitis B, influenza, Rabies, Tetanus Diphtheria (Td), and Tetanus, Diphtheria, Pertussis (Tdap). The contractor must be Kansas state licensed.
