Formalin is shipped out quite often other laboratories and it is imperative that the warehouse keeps several cases on hand to meet demand. The timely and effective delivery of formalin will allow for the uninterrupted shipments to USDA customers. Please see attachment for statement of work and contract terms and conditions. Quotes are due by 7/31/2020 at 1pm central time. Quotes received after this time will not be considered. The contractor shall send the quote via email to the attention of Kim Yen Tu @ kimyen.n.tu@usda.gov. Interested Contractors shall submit the following by the due date to be considered: 1. Contractor SAM Record or DUNS information 2. signed SF1449 and Filled price schedule (page 4 of Statement of Work) 3. Pre- award sample of specimen transport container sent to NCAH (must be received at NCAH by 8/03/2020) 4. Other Information/literature of product being offered (case and packing information)
