AMENDMENT 0001 (Dated 08/05/2024): This amendment (0001) is being issued to (1) extend the quote submission deadline, (2) add a question and answer document to the Attachments/Links, and (3) add an SF-30 solicitation amendment form to the Attachments/Links. All other terms and conditions remain unchanged. This request for quote (RFQ) solicitation # 12505B24Q0287 is to provide Cage Washer in accordance with the Requirements provided in the “Attachments/Links” section. The associated NAICS is 339112 (“Surgical and Medical Instrument Manufacturing”) with a small business size standard of under 1,000 employees. The applicable PSC is 6640 (“Laboratory Equipment and Supplies”). This solicitation is issued as Unrestricted. It is the Government’s intent to make a single firm-fixed price award. Any questions and all quotes must be submitted in writing via email to Aaron Dimeo at aaron.dimeo@usda.gov with the solicitation number (“12505B24Q0287”) and/or title ("Cage Washer") in the subject line, body, or attachments of the email. A completed copy of the SF-30 solicitation amendment form, SF1449 quote submission form, and the supplemental "Supporting Information Form" are to be include with your quote.
