When submitting your response to this RFQ, include Tanya.Linnell@usda.gov in addition to the contract specialist listed in section (xxi) of the RFQ document. All other terms and conditions remain unchanged. Return a signed copy of this amendment with your quote. The United States Department of Agriculture (USDA), Forest Service (FS), Intermountain Region, Fishlake National Forest has a need for supplies for the Pleasant Creek Water System which meet or exceed the specifications contained in the description section of the attached request for quote (RFQ) document. Responses to this solicitation shall include the following: AT A MINIMUM, QUOTERS SHALL SUBMIT THE FOLLOWING DOCUMENTS BACK WITH THEIR RESPONSE TO THIS SOLICITATION: 1. Schedule of Items (including Quoter’s Company Information) (Page(s) 1-2). 2. Any Amendment that may be issued. 3. Exhibit #1 Past Performance Questionnaire (Page 21). 4. FAR 52.204-24(d)(2) Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment (Page 22). 5. FAR 52.212-3(b)(2) Offeror Representations and Certifications – Commercial Products and Commercial Services (Page 23). 6. Technical Response as Identified in Evaluation Factors for Award. (See Evaluations Commercial Items (Page(s) 24-25). Note: In addition to the RFQ document, there is a Material Reference Information Document attached which will aid in identifying specifications.
