If submitting a quote, complete blocks 17a, 30a-30c on page 1, all Schedule of Items listed in Section B for the BASE Year and each Option Year(s) along with the summary tables, and all highlighted fields in the entire solicitation, including Sections B & F. Complete Attachment #4 and provide a list of similar projects completed by your firm over the past three years. Return the entire completed solicitation, with the documentation that addresses that the services your company provides can accomplish work required to be completed listed in Section B of this solicitation.
