This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. DESCRIPTION OF REQUIREMENT Deschutes and Ochoco National Forest are contracting for emergency medical treatments to treat life-threatening allergic reactions. Contractor shall provide 103 (1 package (Contents: 2 Unit or 2 Pak)) EpiPen or Generic Epinephrine Injection, USP Auto-Injectors. Each pen shall deliver a single dose of Epinephrine Injection, USP 0.3 mg Auto-Injection for adults in a sterile solution. Products shall have a shelf life of at least 12 months; 15-18 months is most preferred. Delivery shall ensure appropriate temperate between 15-25°C, but not refrigerated, as temperatures below 15°C may damage injector mechanism. All supplies provided by the Contractor shall be in compliance with all known and applicable Federal, State, and Forest Service regulations and laws, and standard industry practices for these supplies. DELIVERY LOCATION Delivery will be to USDA Forest Service Deschutes National Forest Attn: Ross Scrocca 63095 Deschutes Market Road Bend, OR 97701. DELIVERY DATE The delivery date shall be on or before July 15th, 2022. Request for quotes shall be sent to Amanda Nemeth at amanda.nemeth@usda.gov by COB June 30th, 2022, by 12:00 PM Pacific Time.
