AMENDMENT 04 This amendment provides all interested vendors with company questions asked and Forest Service answers. In addition, the requirement for 2-way communication is removed from Attachment 6 - Fire Protection and Suppression. AMENDMENT 03 Updates the Schedule of Items by amending Line Item No. 0002AE Spot Surfacing (Spec. 813(3)) and adding 0002AL Spot Surfacing (Spec. 813(1)); b. Update Attachment 3 - Project Worklist to reflect the Schedule changes; and c. Update the Period of Performance. Vendors are reminded to be sure and sign and submit all amendments with your quote package submission. AMENDMENT 02 Extend the closing date/time as indicated in SAM based on the CO receiving no "responsive" quotation packages. Vendor reminders: 1) Review the "Evaluation Factors for award' and make sure all required documents are submitted according to the solicitation's requirements; 2) All vendors must have an active SAM account at the time you submit a quotation package; and 3) Submit your quotation package on or before the closing date/time; late quotation packages are non-responsive. AMENDMENT 01 Extend the closing date/time as indicated in SAM based on the CO receiving no quotation packages. DESCRIPTION OF REQUIREMENT US Forest Service Region 6, the Mt. Baker-Snoqualmie National Forest is in need of road maintenance service. These are roads providing access to and within National Forests and other federal lands managed by National Forest programs. They generally include Forest Highways, Forest Development Roads and Parkways. The intent of this performance-based standalone contract is to provide for roadside brushing and road maintenance of forest roads. The Contractor shall furnish the necessary personnel, material, equipment, services and facilities (except as otherwise specified), to perform the Statement of Work/Specifications referenced herein. The scope of work includes common road maintenance activities including the following: grading, brushing, ditching, spot rocking, culvert cleanout, slide and obstruction removal, installing signs, asphalt pothole repair and culvert removal. FAR 52.212-2 Evaluation – Commercial Products and Commercial Services (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
