This is a combined synopsis/solicitation for commercial goods and commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number is 12405B24Q0191 and is issued as a request for quotation (RFQ). The NAICS code is 621111. This solicitation document incorporates those provisions and clauses in effect through Federal Acquisition Circular 2022-06. The purpose of the Occupational Medical Surveillance Program is to provide medical surveillance for all authorized USDA-ARS employees who are exposed to hazards, even just occasionally, that could potentially result in a work-related illness. The results of the medical testing and examination will determine if action is required on the part of USDA-ARS to correct actual or potential health problems, and possibly eliminate exposures. The medical provider shall identify problems and determine what changes may be necessary. Non-personal services for the ARS Occupational Medical Surveillance Program (OMSP), mandated by OSHA, designed to identify exposure to potentially harmful agents and aid in the early detection of adverse health effects in relation to their work and making medical recommendations regarding worker placement, accommodation, exposure controls and medical treatment and advice for any employee Federal working at the USDA ARS Mississippi State Location. Services and equipment to provide blood assays such as CMP, lipid, RBC cholinesterase, PSA, CBC, urinalysis, chest x-rays, audiometry, pulmonary function spirometry and other medical evaluations as requested from evaluating medical professional. Destination), technical and past performance are more important than price. Award will be made based on overall BEST VALUE to the Government. The government anticipates award of a firm fixed priced contract resulting from this solicitation to the responsible offeror whose offer conforms to the specifications that will be the most advantageous to the government in accordance with FAR 13.106-2. Responses shall include technical specifications, descriptive material and capabilities of the offeror’s equipment. Offerors shall identity where the offered item meets or does not meet each of the Government's functional and performance minimum specifications listed herein.
