This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number is 12405B23R0013 and is issued as a request for quotation (RFQ). The NAICS code is 621111. This solicitation document incorporates those provisions and clauses in effect through Federal Acquisition Circular 2022-06. The purpose of the Occupational Medical Surveillance Program is to provide medical surveillance for all authorized USDA-ARS employees who are exposed to hazards, even just occasionally, that could potentially result in a work-related illness. The results of the medical testing and examination will determine if action is required on the part of USDA-ARS to correct actual or potential health problems, and possibly eliminate exposures. The medical provider shall identify problems and determine what changes may be necessary. The vendor should follow their own policies and OSHA’s standards as well. Recommendation: all employees who are to have medical clearance to wear a respirator should have a complete baseline physical exam including spirometry and a baseline PA chest x-ray. Vendor must send RMC information directly to USDA Local Contact. Note that the USDA Local Contact is to receive notification of clearance or no clearance ONLY.
