This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number is 12405B23Q0172 and is issued as a request for quotation (RFQ). The NAICS code is 621111 – Offices of Physicians. This solicitation document incorporates those provisions and clauses in effect through Federal Acquisition Circular 2022-06. The USDA, ARS Gainesville, FL location requires various medical tests for USDA employees throughout the year, to include the following: Physical, CBC, Liver Test, Kidney Function, Urinalysis, Pulmonary Function Test (Spirometry), Chest X- Ray, Audiogram, Reticulocyte Count, RBC Cholinesterase, Plasma Cholinesterase, Thyroid Test, Respirator Fit Test (Quantitative), Respirator Fit Test (Qualitative), and OSHA Respirator Questionnaire. Vendor shall be able to offer ALL required testing services to be approved at one (1) location. Testing location needs to be within reasonable driving distance from 1600 SW 23rd Dr. Gainesville, FL The requirement is for up to 25 patients and up to 1 of each test per patient. The period of performance will be for a base year and four option year periods. Any quotes not meeting the minimum specifications will not be considered. The Service Contract Act Wage Determinations will apply to this contract. The government anticipates award of a firm fixed priced contract resulting from this solicitation to the responsible offeror whose offer conforms to the specifications that will be the most advantageous to the government in accordance with FAR 13.106-2. Responses shall include technical specifications, descriptive material and capabilities of the offeror’s equipment.
