The nature of this action is to extend the existing Basic Ordering Agreement (BOA) to Care Meridian, LLC d/b/a CareMeridian for one (1) year, effective 10/24/2020 10/23/2021, for Community Nursing Home (CNH) services. The incumbent specializes in residential care and rehabilitation services to Veterans with brain injuries, dementia, and other neurologic disorders; to included, but not limited to: multiple scoliosis, Parkinson s, amyotrophic lateral sclerosis (ALS), chronic obstructive pulmonary disease (COPD), respiratory failure, and medically complex cases with multiple comorbidities. This sole source action is in accordance with FAR 6.302-1. Only One Responsible Source and No Other Support or Services Will Satisfy Agency Requirements. THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSAL.
