Sources Sought Notice The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of SDVOSB, VOSB, SB and Larges Business that can provide Culligan Preventative Maintenance of Water Treatment Equipment as below. DEPARTMENT OF VETERANS AFFAIRS MEDICAL CENTER Truman VA, Columbia, MO STATEMENT OF WORK Industrial Water Systems Preventative Maintenance 1. Introduction: This Statement of Work (SOW) describes the requirements for depending on the equipment for monthly, semi-annual, or annual preventative maintenance of the water treatment equipment listed in Appendix A. The Appendix B breaks down the maintenance/services to include the frequencies to be provided on the corresponding equipment at Harry S. Truman Memorial Veterans Hospital (HSTMVH). This requirement is for base with four option years. Work shall be performed during business hours, 0730-1600, Monday-Friday. Variations to this schedule may be possible for certain equipment when coordinated with Contractor Officer Representative (COR) which will be designated once awarded. All work shall be coordinated with COR. Contractor shall schedule work two (2) weeks in advance to assure proper coordination with hospital personnel. Contact the designated COR to schedule testing. 2. Qualifications of Contractors: Contractors shall have technical training and a history of work experience in maintenance, inspection, and testing of industrial water systems and its components in healthcare, industrial, educational, and commercial facilities for a minimum of five (5) continuous years. B. Contractors shall furnish all necessary tools, equipment, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Tools, equipment, and PPE shall comply with the requirements of OSHA Standard 29 CFR 1910, Subpart I. 3. General Work Items: Equipment and/or components to be covered under this service contract are listed in Appendix A. Appendix B breaks down the contracted services to include the frequencies services are to be provided on the corresponding equipment. Emergencies- Additional Services: All work must be coordinated and approved by COR before emergency/additional work is started or contractor will be responsible for work completed. Additional emergency or service visits are billed at the following rates: Normal Business Hours: $______/hr. 1 hour minimum Monday through Friday 8:00 a.m. to 4:00 p.m. After Normal Business Hours: $______/hr. 1 hour minimum Weekends: Saturdays: $______/hr. 2 hour minimum Sundays: $______/hr. 2 hour minimum The above rates shall include travel time to and from the system location. Additional time on site above the minimum, will be billed in increments of full hours only. All parts needed on a break/fix scenario will be discounted at 20% off Culligan listed prices and the discounted labor rate above will be honored. 4. Reporting: Provide complete written report of the work performed. Information shown in the report shall be as follows: Company s name, address, telephone & FAX numbers. Name and signature of contractors who perform the maintenance and testing. VA Work Contract Number, name and number of VA Contracting Officer. Date and time of work. Copies of contractors valid licenses, professional and training certificates. Descriptions and model number of specialized tools and equipment used. Location and type of equipment serviced. Descriptions of work items. Recommended corrective actions, if any. Contractors shall report deficiencies that are deemed critical or catastrophic immediately to the Contracting Officer Representative (COR) for immediate actions. 5. Customer Responsibility: Customer s responsibilities for providing continual maintenance of the system. 1. Monitor the system daily and record the operating parameters as required in the O & M manual logs. 2. Notify Culligan by the next business day of any malfunction or operation of the system outside normal operating conditions, as specified upon start-up. 3. Provide for and supply all necessary utilities. Notify Culligan by the next business day, of any change in feed water characteristics or the volume of water used. 4. The customer will either provide the labor and/or materials or add additional funding to existing Purchase Order for: a. To change expendable DI cartridges, service DI, and filter, if required between scheduled PM service visits. b. Any additional sanitizations, if required, between scheduled PM service visits. c. Any repairs that are not specifically covered on Schedule B. 5. Provide a minimum of 4 hours down time of the system, for Culligan to perform each scheduled PM service visit. 6. The Customer is suggested to have an inventory of spare parts that are considered typical wear parts. Please consult the manual for a list of suggested spare parts. 6. Contractor must comply with the following COVID 19 Guidance. 52.223-99 Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors. ENSURING ADEQUATE COVID-19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS (OCT 2021) (DEVIATION) (a) Definition. As used in this clause - United States or its outlying areas means (1) The fifty States; (2) The District of Columbia; (3) The commonwealths of Puerto Rico and the Northern Mariana Islands; (4) The territories of American Samoa, Guam, and the United States Virgin Islands; and (5) The minor outlying islands of Baker Island, Howland Island, Jarvis Island, Johnston Atoll, Kingman Reef, Midway Islands, Navassa Island, Palmyra Atoll, and Wake Atoll. (b) Authority. This clause implements Executive Order 14042, Ensuring Adequate COVID Safety Protocols for Federal Contractors, dated September 9, 2021 (published in the Federal Register on September 14, 2021, 86 FR 50985). (c) Compliance. The Contractor shall comply with all guidance, including guidance conveyed through Frequently Asked Questions, as amended during the performance of this contract, for contractor workplace locations published by the Safer Federal Workforce Task Force (Task Force Guidance) at https:/www.saferfederalworkforce.gov/contractors/. (d) Subcontracts. The Contractor shall include the substance of this clause, including this paragraph (d), in subcontracts at any tier that exceed the simplified acquisition threshold, as defined in Federal Acquisition Regulation 2.101 on the date of subcontract award, and are for services, including construction, performed in whole or in part within the United States or its outlying areas.
