Disposable Sleep Study Supplies and Services for Dayton VAMC The purpose of this sources sought notice is to seek responses from all qualified, prospective offerors interested in the forthcoming solicitation for the subject requirement summarized as follows: Short Title: Disposable Sleep Study Supplies and Services for Dayton VAMC NAICS-621610: Home Health Care Services Place of Performance: Patients home of residence in support of Dayton VA Medical Center (VAMC) Anticipated Periods of Performance: Base Year: 16 December 2021 to 15 December 2022 Option Period One (6-months): 16 December 2022 to 15 June 2023 Option Period Two (6-months): 16 June 2023 to 15 December 2023 Option Period Three (6-months): 16 December 2023 to 15 June 2024 Option Period Four (6-months): 16 June 2024 to 15 December 2024 Attachments for Reference: A draft version of the Performance Work Statement (PWS) is attached below for offeror reference as appropriate.
