In Virginia the Bristol VA Clinic, Bristol, VA, Norton VA Clinic, Norton, VA, Marion VA Clinic, Marion, VA, Vansant VA Clinic, Vansant, VA, and the Jonesville VA Clinic in Jonesville, VA is conducting market research through this sources sought notice to seek sources to identify potential experienced businesses that are capable of meeting the requirements per the Statement of Work (SOW) below Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 561740 and sized standard is $7.5. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. Anticipated period of performance: 12/01/2022 thru 11/30/2027. General Information Statement of Work Title of Project: Mat Service for VAMC Mountain Home, TN and VA outpatient clinics as listed. In Tennessee James H. Quillen VA Medical Center, Johnson City, TN, William C. Talent (aka: Ray Mears / OPC), Annex 1, Annex 2, and Annex 3 in Knoxville, TN. Dannie A. Carr Veterans Outpatient Clinic in Sevierville, TN, Morristown East VA Clinic in Morristown, TN, Mountain City VA Clinic in Mountain City, TN, Campbell Co. VA Clinic in Lafollette, TN, and the Rogersville VA Clinic in Rogersville, TN. In Virginia the Bristol VA Clinic, Bristol, VA, Norton VA Clinic, Norton, VA, Marion VA Clinic, Marion, VA, Vansant VA Clinic, Vansant, VA, and the Jonesville VA Clinic in Jonesville, VA. Hours of Operation: Hours of operation will be included in the table on this SOW. These hours are subject to change. It will be the contractor s responsibility to deliver Mats when the clinics are open and adjust their schedule as needed. AT NO POINT WILL MATS BE LEFT OUTSIDE FOR CLINIC EMPLOYEES TO SWITCH OUT. SCOPE OF WORK: The Contractor shall provide ALL RESOURCES to include the appropriate number of mats necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Mat service is needed for VA Medical Center Mountain Home and the surrounding area outpatient clinics for a weekly pickup and delivery of clean, well serviced mats. (MATS THAT ARE TORN, DIRTY, THAT DO NOT LAY COMPLETELY FLAT, OR UNSERVICABLLE IN ANY WAY WILL NOT BE ACCEPTABLE) Required Contractor Reporting: Upon arrival to James H. Quillen VA Medical Center the Contractor will sign in with the COR in Bldg. 8 Room 117 (423-926-1171 x/7889) and must sign out at the completion of service each day. Contractors shall report to office managers when deliveries are made to the outpatient clinics. Interference to Normal Function: Contractor personnel are required to interrupt their work at any time so as not to interfere with normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment, and carts. Contractor Badges and Parking: 1. While on VA premises, all contractor personnel shall comply with the rules, regulations, and procedures governing the conduct of personnel and the operation of the facility. 2. It is the responsibility of the contractor s personnel to park in the appropriate designated parking areas. 3. James H. Quillen VAMC does not validate or make reimbursement for parking violations 8. IT Security: The certification and accreditation do not apply and a Security Accreditation Package is not required.
