Statement of Work Attached Wage Determination-Attached Page 1 of 6 COMBINED SYNOPSIS/SOLICITATION The Miami VA Medical Center located at 1201 NW 16 Street, Miami, FL 33125-1477 has a requirement to provide preventative maintenance services for HillRom beds for a Base plus Four (4) Option Years. The attached Statement of Work (SOW) shall be adhered to. This is a combined synopsis/solicitation, Request for Quotation (RFQ), for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The applicable North American Industry Classification System (NAICS) Code and size standards in millions of dollars are 811219 and $22.0, respectively. FAR 52.212-2, Evaluation - Commercial Items is applicable to this procurement. This requirement will be awarded to a responsible contractor whose proposal represents the best value to the Government, considering price and technical factors, with technical and past performance factors more important than price. The evaluation of offers received in response to the solicitation will use a tiered or cascading order of precedence. Tiered evaluation of offers, also known as cascading evaluation of offers, is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. The contracting officer (1) Solicits offers from both small and other than small business concerns that will be evaluated in the following tier order: (a) service-disabled veteran-owned small business (SDVOSB); (b) veteran-owned small business (VOSB); (c) all other small business concerns; (d) other than small business. (2) If an award or a sufficient number of awards cannot be made at the first tier, evaluation of offers will proceed at the next lower tier until an award or a sufficient number of awards can be made. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Contractor Statement of Work HILLROM bed Preventative Maintenance and service contract for Smart care protection plus and complete service VA Medical Center Miami, FL. General Requirements The government requires a contract be negotiated for a full-service preventative maintenance program and bed service program between the government and the bed manufacturer HILLROM. The government requires complete service. PROJECT OVERVIEW: The following shall be included in the Smart care protection plus and complete service a. Parts and labor required for maintenance and repairs. b. preventative maintenance (PM) will be performed as outline in the original equipment manufacture (OEM) technical service manual. c. Customer must provide all parts and service manuals for maintenance and repair of non-Hillrom brand equipment. d. Ongoing user assistance while Hill-Rom s service technician is on site. e. Customer access to document of performed services at my.hillrom.com f. Protection plus plan is available at time of capital purchase up to 90 days after product ship date. SPECIAL REQUIREMENTS VA Police will issue identification badges to contractors, employees and subcontractors which must be worn at all times when on VA Property. Contractor shall provide a list of all personnel who will participate on the project. Personnel shall be subjected to a background investigation previous to commencement. After background check has been completed identification badges will be issued. Contractor shall keep the area cleaned daily and haul away all construction debris. All materials and construction debris must be in covered cart when entering and exiting the building. To minimize the impact to patient, care it is possible the work might have to be completed during nights and weekends and will need to be coordinated and approved by the COR. Contractor will have approval from the COR and engineering staff on the installation of any temporary systems for the medical gas farm systems. Provide a weekly production schedule. Prior to any work starting a materials list will be provided to the COR for review and approval. QUALIFICATION / CERTIFICATION / RESPONSIBILITIES D/B Additionally, the contractor shall comply with the following requirements: Develop a plan detailing project specific fire-safety measure. Maintain site and building access free and unobstructed to facility emergency services. Means of egress shall not be blocked. Egress routes for construction workers must be maintained free and unobstructed. Contractor furnished fire extinguishers must be provided and maintained in construction areas and in temporary storage areas. Flammable and combustible liquids must be stored, disposed and used in accordance with OSHA 1926, NFPA 241 and NFPA 30. Existing fire protection must not be impaired. Provide fire watch for impairments more than four (4) hours in a 24-hour period. Prevent accidental smoke detector operation. Perform Hot Work in accordance with NFPA 241 and NFPA 51B. Maintain all construction permits visible and updated as required by the MVAMC Safety Office and Infection Control Department. Prohibit smoking in and adjacent to construction areas. Dispose of waste and debris from buildings DAILY. Follow all additional requirements as specified in the MVAMC Construction Policy. EQUIPMENT All Equipment, tools, etc., shall be maintained while on the project and shall be adequate, in quality and capacity for proper execution of work. Under no circumstances tools or equipment shall be left unattended. SCHEDULING AND HOURS OF WORK All work in areas involved shall be scheduled with the VA Engineering COR. Work shall be scheduled for proper execution to completion of this contract. Hours of Work: The contractor will perform work from 7:30 am 4:00 pm Monday thru Friday. Evening and week hours will be coordinated with the COR. No work shall be performed during U.S. Federal Holidays. Normal duty hours are 7:30 am 4:00 pm Monday thru Friday except, U.S Federal Holidays. INSPECTIONS All work shall be inspected daily, at final completion by MVAMC COR. SAFETY REGULATIONS All applicable Safety Regulations shall be observed during all work of this contract. The MVAMC Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. Provide all necessary barricades / signage where required. Isolate the area to prevent the spread of debris into adjacent area. Follow Infection Control regulations. Maintained the area negatively pressurized if required. Perform field test and participate on final acceptance tests for new equipment or systems installed. Provide a written certified statement indicating that the construction project meets the life safety code requirements. For those maintenance and repair projects where the life / safety code requirements are not applicable, a written statement shall also be provided accordingly.
