The Beckley VA Medical Center is conducting this Sources Sought to identify contractors who possess capabilities to provide the services described in the Statement of Work below. As permitted by the FAR Part 10, this is a market research tool being utilized for the informational and planning purposes. Your responses will assist the Government in the development of its acquisition strategy. Response is strictly voluntary it is not mandatory to submit a response to this notice to participate in formal RFQ process that may take place in the future. However, it should be noted that information gathered through this notice may significantly influence our acquisition strategy. All interested parties will be required to respond separately to any solicitation posted as a result of this sources sought notice. Respondents will not be notified of the results of the evaluation. This service will be implemented using the North American Industry Classification System (NAICS) Code 811219. If a small business is interested in this procurement, they must provide at least 51 percent of the services rendered. Responses to this Sources Sought Notice must be received no later than January 5, 2022 at 12:00 p.m. (Eastern Standard Time). This Sources Sought Notice may or may not assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued, it may or may not be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this Sources Sought Notice. Responses to these notices are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses incurred associated with responding to this RFI. SCOPE OF WORK Audiometric Booths GENERAL: This service shall consist of the manufacture, delivery, and installation of three new audiometric booths that will be located in the new Audiology Department on the first floor of Building 1. LOCATION: This service is on the first floor, Building 1 at the VA Medical Center in Beckley, West Virginia. PROJECT DESCRIPTION: This scope of work (SOW) is intended to provide the prospective audiometric booth supplier with a description of work to be completed. This specification addresses prefabricated audiometric booths and rooms/suites suitable for use in testing, calibration, and recording of aural acuity. A booth is defined as a free-standing examination room. Henceforth, the term audiometric booth will be used generically when referring to booth, room, or suite. An audiometric booth covered by this specification includes all enclosure panels, components, wiring, lighting fixtures (including dimmers), ventilation silencers, and installation to make the booth completely operable and 100% compliant with the VHA Handbook 1170.02 Appendix D. The audiometric booths are to fill the space available in the plan view (CAD Drawing) attached, wrapping around existing columns. The audiometric suite installation shall conform to all the provisions of the contract documents. Work will be completed in an active hospital and may require phase installation planning. The Contractor will deliver and install all the audiometric suites and be responsible for storage of materials/audiometric suites in a bonded warehouse until the space is ready for installation. The Contractor is solely responsible for damage to facilities resulting from delivery and installation activities. This shall include cleaning and repairs of walls, floors, corridors, elevators, and any other parts of the facility to the satisfaction of the Contracting Officer s Representative (COR). At the conclusion of the installation, a post- installation walkthrough will be performed. If damage is identified, a punch list will be established. Prior to acceptance, the COR shall inspect adjustments made by the Contractor to satisfy the punch list items. The Contractor will ensure that all sub-contractors meet and abide by all government regulations and requirements. Work shall be accomplished Monday through Friday, 8:00 am to 4:30 pm. Unless directed by the Contracting Officer (CO), work at the site(s) shall not take place on Federal holidays or weekends. When a holiday falls on Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed by the U.S. Government agencies. Veteran's Day 10. Thanksgiving 11. Christmas The information provided in this SOW includes but is not limited to the following: AUDIOLOGY DEPARTMENT, 1ST FLOOR BUILDING 1 1. Audiometric booth dimensions: See drawing attached. Size as required to fit in space, wrapping around existing columns. Audiometric booth contractor to provide architectural drawings showing that the audiometric booths adequately fit into the designated space. 2. Booth contractor to finalize number of electrical outlets and other jacks/inputs/ports within the audiometric booth. 3. Booth contractor is required to verify all dimensions of the space allocated for the audiometric booths. 4. Audiometric booths are expected to be delivered within 8-10 weeks of award. 5. A two-week notice shall be given as to the installation date of the audiometric booths. 6. Delivery shall be coordinated with the VA and the VA contractor for project, Replace Records Room with Audiology. All correspondence that occurs between the booth contractor and VA contractor for project, Replace Records Room with Audiology will take place through the VA COR. 7. Installation of audiometric booths shall be completed by booth contractor and be provided by individuals certified on booth installation process. 8. The audiometric booths shall be constructed of modular, prefabricated panels and assembly hardware. 9. The booth contractor shall coordinate with VA contractor for project, Replace Records Room with Audiology on the locations of the sprinkler, all electrical components, and all HVAC components in the audiometric booths. 10. Audiometric booths shall be independent, free standing structures, that provide all required structural support for the enclosure as part of the design. 11. Audiometric booths will be manufactured so that it can be disassembled, relocated, and re-assembled in the future, without compromising the integrity of the booth. 12. Inspection of material must be done prior to installation. 13. Inspection must be done after completion of installation to insure the following: a. that the floors are level, and the walls are plumb. b. That the booth has proper paint coverage with no dents or scratches. Any finish defects shall be repaired by the contractor. c. That the audiometric booth doors are properly aligned and swing freely without binding other than as necessary for proper sealing. 14. VA shall determine all color choices from entire color palette. 15. Furnish with each booth, two copies of equipment manual containing installation, operation, and maintenance instructions. This shall include a parts data section to include a list of all parts with replacement instructions. 16. Testing shall be conducted to determine that all booths meet acoustical performance standards. Booth contractor will be required to remove and insure proper disposal of all packing and shipping materials. Booth contractor will be working around an occupied healthcare facility in operation 24 hours a day and care will be taken to keep work areas clean and contained.
