Nizhoni Blvd, Gallup, NM 87301 is seeking capable sources for Non-Personal Service Contract for the following Provider Specialties: 1. Anesthesiologist 2. Certified Registered Nurse Anesthetists (CRNAs) 3. General Surgery 4. Internal Medicine - Hospitalist 5. Podiatry 6. Orthopedics The anticipated period of performance is 04/01/2024 through 03/31/2025 (Some specialties may need to start soon as possible). See below for full service performance: Base Period: 04/01/2024 through 06/30/2024 1st Option Period: 07/01/2024 through 09/30/2024 2nd Option Period: 10/01/2024 through 12/31/2024 3rd Option Period: 01/01/2025 through 03/31/2025 ***Period of Performance date may be subject to change*** The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 621111 with a small business size standard of $16.0 Million. The government will evaluate market information to ascertain potential market capacity to provide supplies with those described in this notice. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises.
