This is a combined synopsis/solicitation for Non-Personal Service Contracts for various Registered Nursing Departments prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. The solicitation number RFQ1464304B is issued as a request for quotation (RFQ). This Request for Quote (RFQ) is for a Non-Personal Service Contract for a Contractor to provide Registered Nurse healthcare providers to the Gallup Indian Medical Center, Gallup, New Mexico of the Indian Health Service (IHS) throughout the duration of the contract period of performance. The Gallup Indian Medical Center has the designation as a Level III Trauma Center. Services shall be performed in accordance with the attached Performance Work Statement (PWS – Attachment No. 2). The Navajo Area Indian Health Service (IHS), Gallup Service Unit (GSU) intends to award a firm-fixed-price contract. The Government reserves the right to not make an award, if necessary. Period of Performance: The Government will award a firm-fixed-priced contract for one (1) Base Period and Three (3) Option Periods: Base Period: Date of Award through 03/31/2021 First Option Period: 04/01/2021 through 06/30/2021 Second Option Period: 07/01/2021 through 09/30/2021 Third Option Period: 10/01/2021 through 12/31/2021 The Contractor shall provide an all-inclusive rate, and is responsible for all costs associated with providing said services, i.e. travel, per diem, lodging/housing, etc. The Contractor shall be responsible for housing respective providers as there are no government quarters available. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-01, effective October 01, 2020. The RFQ is issued with the NAICS code 561320 – Temporary Help Services, with a small business size standard of $30.0 dollars. Attachment No. 1 - Pricing Schedule Attachment No. 2 – Performance Work Statement Attachment No. 3 – Statement of Duties To be considered for award, your offer shall include the following documents: 1. PROVIDE A RATE THAT WILL ALLOW YOU TO RECRUIT PROVIDERS THROUGHOUT THE CONTRACT PERIOD. The Government will consider competitive rates upon negotiation, and will obligate funding on a quarterly basis. 3. Provide complete Candidate Profile: Curriculum vitae, resumes, licensures, and all relative documents for proposed candidates. 4. Provide evidence of Provider's Experience of comparable scope and complexity in providing services within the past three (3) years. 5. Provide evidence of the Contract Company's Past Performance of comparable scope and complexity in providing services within the past three (3) years. Evaluation Criteria 1. The Government will award a contract resulting from this solicitation to the best value Offeror. The following factors shall be used to evaluate offers: 2. Technical Capability – Provide evidence of trained and certified personnel, technical support and field engineers capable of servicing equipment. 3. Past Performance – Provide evidence of performing similar scope within the past three (3) years. 4. Logistics – Provide evidence of contractor capability of responding to scheduled service within two (2) hours of contact (Telephone) and a service call shall be resolved within a minimum of 24 hours.
