Annual Generator and Load Bank Service Gallup Indian Medical Center (GIMC), Gallup, NM 87301 Statement of Work DESCRIPTION: The Contractor shall provide all labor, equipment and personnel to perform Annual Level 2 preventative maintenance and load bank of three emergency generators at the Gallup Indian Medical Center (GIMC). All tests shall meet or exceed current Joint Commission Accreditation requirements. DELIVERABLES: Contractor will provide complete Level 2 service for the following generators: Gen Set 1: Model# 750DQFAA-6499 Serial# B070027417 Gen Set 2: Model# 750DQFAA-6499 Serial# B070027416 2018 Gen Set: Model# DFHB-5634228 Serial# K030573664 Contractor shall load bank each generator as follows: 50% of name plate rating for 30 minutes 75% of name plate rating for 60 minutes. Run time will be a total of 1 ½ continuous hours. GENERAL CONDITIONS: Codes and Standards. The Contractor shall adhere to all emergency generators tests and inspections shall be in accordance with the following: 2012 Edition NFPA 101 – Life Safety Code 2010 Edition NFPA 110 – Standard for Emergency and Standby Power Systems Joint Commission Standard EC.02.05.07 EP6 Safety: Contractor shall supply all worksite safety equipment and practices. Any damages made by the contractor shall be reported immediately to the Contract Officer and Contract Officer’s Representative. Contractor will be liable for any damages. Contractor will be responsible for complete and strict compliance with all Gallup Service Unit facility policies, including safety policies and Pre-Construction Risk Assessment (PRCA). Contractor shall provide influenza (flu) Shot at their own expense to their personnel and provide Flu Shot documentation to COR or designee upon issuance of Notice to Proceed. This is in compliance with Indian Health Service mandatory influenza policy. Contractor(s) will sign in and out at Security each day with issuance of visitor’s badge. All Contractors’ personnel will be accompanied by GIMC Maintenance personnel at all times. Quality Assurance All generator service and load bank shall be performed by a contractor specialized in the specific trade required. Execution All activities shall be coordinated with Gallup Indian Medical Center local POC to ensure that there are no detrimental impact on the day-to-day operations of the hospital. Inspection Government reserves the right to inspect all aspects of work performed. Acceptance based on complete test and inspection of all three generators and load bank. Documentation shall include: Test and inspection reports. Load Bank test reports Schedule: Contractor shall perform work under this contract only during the time outlined below. Monday through Friday, 8AM – 5PM, excluding federal holidays. Work outside these hours must be submitted within 72 hours in advance in writing to the Contract Officer for approval. FEE SCHEDULE: Contractor will indicate below the Total Cost to perform the stated services. Total costs shall include labor, travel, per diem, tools and equipment to perform all necessary service, tests and inspections. Include line item for Taxes on Labor only PERIOD OF PERFORMANCE: The period of performance: Base Year: November 1, 2021 – October 31, 2022 Option Year One (1): November 1, 2022 – October 31, 2023 Option Year Two (2); November 1, 2023 – October 31, 2024 Option Year Three (3): November 1, 2024 – October 31, 2025 Option Year Four (4): November 1, 2025 – October 31, 2026
