THIS IS NOT A SOLICITATION The Indian Health Service at Four Corners Regional Health Center, Red Mesa, AZ has an upcoming requirement for 2 (two) Blanket Service Agreement for Miscellaneous Dental Supplies for the Dental Department. The Period of Performance is twelve (12) months from date of award plus four (4) Option years. The North American Industry Classification System or NAICS code is 339114 – Dental Equipment and Supplies Manufacturing with a small business size standard of 1,000 employees. The Request for Quotations No. 75H71025Q00064 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by January 31, 2025. Evaluation factors may include the quoter’s technical approach to meet or exceed the government’s requirement as described in the Statement of Work, narrative descriptions of a minimum of three (3) past projects of similar or same complexity within the past five (5) years, and pricing information in sufficient detail. Quotes will be due on or by January 31, 2025. The acquisition is a Total Indian-own, Small Business, Economic Enterprise (ISBEE) set-aside.
