Time: 10:00 AM Tuesday July 27, 2021 Where: We are meeting at the corner of E North Drive road and Sycamore Street Fletcher, Oklahoma. In Fletcher go to North Drive Road, go East on North Drive to Ozark Trail/Sycamore Street. The property location is: turn north and go about ½ mile mobile home is on the west side of the road. This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-21-Q-0098. This procurement is 100% small business set-aside under NAICS code 237110 with a size standard in millions of dollars $39.5. Contractor shall provide a firm, fixed-price to furnish and install a water service main extension in accordance with the statement of work and drawings at Fletcher, Oklahoma. The period of performance for this requirement is 45 calendar days from Notice to Proceed. Site visit is scheduled for July 27, 2021 at 10:00 am CST. We are meeting at the corner of E North Drive Road and Sycamore Street Fletcher, Oklahoma. Questions are due no later than August 1, 2021 at 5:00 pm CST. The closing date for receipt of quotes is August 8, 2021 at 6:00 pm CST. The offer shall be received by e-mail to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote B) Technical specifications of the Statement of work and Capability Statement. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS.
