Amendment 0001 Pre-Proposal Conference Slides Pre-Proposal Conference Contractor Sign-In Pre-Proposal Conference Government Sign-In Pre-Proposal Conference Question and Answer Document Pre-Proposal Site Visit Tour Document -Main MTF Pre-Proposal Site Visit Tour Excel Document-Outlying buildings Revision 1-Attachment 3 - Price Matrix - 13 Dec 19 Request for Proposal (RFP)/Solicitation to provide non-personal healthcare environmental cleaning services (Types I thru VI) and related services in support of General Leonard Wood Army Community Hospital (GLWACH), Fort Leonard Wood, Missouri in support of multiple Medical Treatment Facilities in the Health Service Area of GLWACH, Fort Leonard Wood, Missouri. Services shall include all personnel, training, labor, equipment, tools, supplies, managmenet, administration and services, except as may be expressly set forth as goverment furnished, and otherwise do all things necessary to perform and provide the work efforts described in the RFP. The North American Industry Classification System code for this solicitation is 561720. The small business size standard is $19,500,000.00. This requirement is being solicited as an 8(a) small business set-aside under other than full and open competition in accordance with Federal Acquisition Regulation Subpart 6.204(a). This requirement is performance based and will be awarded using Performance-Price Trade-off evaluation criteria, under which an Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract will be awarded with the issuance of Firm-Fixed Priced (FFP) Task Orders. An IDIQ contract is the optimal contract type for this effort as an exact quanity and/or appropriate categorization of services (Types I thru VI, etc.) cannot be determined on an MTF-specific, individual room basis at the time of contract award. The resultant FFP task orders issued against the IDIQ will clearly describe GLWACH MTF-specific healthcare environmental cleaning requirements and services to be performed and provide for prices that will not be subject to an y adjustment on the basis of the 8(a) contractor's cost experience in performing this contract. All services shall be FFP and be subject to the Service Contract Labor Standards (41 U.S.C. 351) . This solicitation and subsequent contract will include contract line item numbers (CLINS) with a five-year ordering period. The anticipated IDIQ contract will obligate the government to the minimum guarantee of $1,500.00 for the post award conference. The entire amount of the minimum guarantee will be obligated against Fiscal Year (FY) 20 funds. Further obligations will be charged to the FY in which the order is placed. Healthcare enviromental cleaning services are required for 60 months starting 1 September 2020 through 31 August 2025. All elements of the RFP shall be submitted by the 8(a) offeror with a proposal no later than the date/time shown in Block 8 of the Standard Form 1449, Solicitation/Contract Order for Commercial Items. The square feet (SF) for cleaning each MTF under the GLWACH HSA is varied; however, the total estimated SF for the GLWACH HSA is 408,371.
