Attach_2_SLS_Resume_Format_Key_Personnel_rev1 3. Attach_6_Price_Proposal_Workbook_rev1: Tab A - NGA Washington-St Louis; offerors shall only submit rates and totals for base and option periods 4. Attach_10 – Compliance Checklist_rev1: corrected inconsistencies for the format of documents MS Word/PDF 5. Attach_11_Industry_Q&A_rev1: added to incorporate the Industry Q&A responses 6. RFP Section 4.0 Instructions to Offeror – clarification revisions in response to industry Q&As include: a. 4.1.2 Page Limitations – format of documents b. 4.3 Volume I Technical (Factor I) - Sub-Factor 1.2 Key Personnel Resumes (s) – One (1) resume per key position at each location c. 4.5 Volume III – Price (Factor) 3 – FTE and core hours 7. RFP Section 5.1.2.1 Factors and Sub-factors and Relative Order of Importance: revised language. All interested offerors shall submit proposal in accordance with the attached solicitation amendment 0005 section 4 and 5 and attachments. All offerors shall include signed solicitation amendments 0001, 0002, 0003, 0004 and 0005 front page to their proposal submittal as acknowledged of those amendments. Q&As are now close. All other terms and conditions remained unchanged and in full force.
